Are you accepting new patients?

    • Yes! Please contact us to make an appointment, or ask your physician about referring you to our clinic.

How do I get my physician to refer me to Lifestyle?

    • If your physician is not already among the physicians who use Lifestyle Physical Therapy, you can have them contact us about becoming a referring physician. Please give your physician our contact information, and we will be happy to speak with them.

What types of insurance do you accept?

    • We accept most forms of insurance, including Medicare.

What are your hours?

    • We are open Monday through Thursday from 8am until 7pm and on Friday from 8am until noon.

Do you currently have any job openings?

    • We regularly seek people who can contribute to our company. Feel free to send us your resume and if we have an opening for you, we'll give you a call.

What should I expect at the first session?

    • The first session will include an evaluation by a licensed physical therapist where your injury will be assessed. An initial treatment plan will be formed based on the initial evaluation.
    • The physical therapist will design a treatment plan according to each individual patient’s needs and goals.
    • The first session will last approximately 1 hour.

What can I expect from additional sessions?

    • Each session will consist of a combination of exercises in conjunction with some hands on treatment and/or modalities with the ultimate goal of educating the patient in order to implement a reasonable and realistic independent home exercise program to continue improving and maintain overall health and condition.

Do I need to have a prescription in order to make an appointment?

    • The state of Illinois requires a documented referral from a physician such as: primary care physician, specialist, or chiropractor.

What should I wear to my appointments?

    • Comfortable workout clothing is recommended.
    • Upper Extremities: tank tops or t-shirts that are loose fitting and allow the therapist to have access to the injured body part.
    • Lower Extremities: shorts or sweatpants that are loose fitting and allow the therapist to have access to the injured body part.
    • Athletic shoes are recommended.

Will my referring physician know about my progress while in therapy?

    • At the duration of your current script, our therapists send a written progress note to your referring physician which coincides with your follow-up visit that will update them on your current status.
